Crap

I've got a letter from the Mental Health service asking me to fill in a self-assessment form before they'll give me an appointment. I hate filling in things like that cos I never know what to put.

This is also highly annoying cos this was all covered in the letter Nicola sent, the whole purpose of which was to give them this information!

And the fact that the letter says 'if you are unable to complete the form please contact us by phone to let us know' makes me want to bash my head against something.
